Is there someone available to assist me 24 hours a day, 7 days a week?
Yes, our office number is monitored on a 24-hour basis. If you call after hours or on a weekend, simply leave a message and a staff person at Hand To Heart
will get back with you ASAP.
Is there a minimum number of hours of home care service per day?
Yes, we have a 4hr. minimum per day. We do ask our clients to be flexible when requesting short hours of service.
Do I need to sign a contract with the agency?
Yes.
Do you accept insurance?
Yes, we accept most long-term care insurance plans.
Do you accept Private Pay?
Yes, we can accept payment(s) directly from a client, client representative, family member on behalf of the client, and an agency on the behalf of a client.
How do I file the claim?
We do all the claims and billing for you.

Yes. A staff person at Hand To Heart
will help you determine what your service needs are and how to best meet them.
How will I know what my services are?
You will receive a copy of the service plan listing your services, dates, times, and other important information after your assessment but before the starting of a caregiver.
How often will someone from Hand To Heart check-in face-to-face?
We make supervisory face-to-face visits every 3 months and a supervisory phone call once monthly. If check-ins are needed more often, we will schedule them.
Will you coordinate my, care with my doctor?
We gladly coordinate your care with your doctors, case managers, social workers, and/or any family members you give us permission to speak with on your behalf.
Do the caregivers provide transportation?

Can caregivers drive me around for appointments and errands.
Yes, Caregivers may use their own personal vehicle to transport a client. Caregivers may also drive a client’s vehicle under certain conditions. We are glad to discuss either option with you if transportation is a service you may need.
Will the same caregiver assist me every day or will it be somebody different?
We strive for continuity of care. We make every effort to keep you with the same caregiver to promote consistency.
How do I make changes to the initial caregiver schedule?
You may call and speak to a staff person at Hand To Heart
or simply make those changes directly with the caregiver.
How can I cancel the service I requested?

and we will be glad to discuss any service(s) cancellations or modifications.
