At Hand To Heart we provide an arrangement of supportive care services with the goal of enabling the client to remain at home. Caregivers may follow a client to other homelike settings like Nursing Homes, Hospice, Assisted Living Facilities, and more. Our caregivers are highly trained to provide care ensuing safety, dignity, and respect and held as the upmost importance. Listed below is a wide range of services that we offer. Anything not listed, we will happily discuss to incorporate it into your plan of care.
Home Care Services:
- Homemaker Services: Our caregivers perform household chores that include housekeeping, meal planning and preparation, shopping assistance, and routine household activities for an elderly, handicapped, or convalescent individual. Our caregivers perform the functions generally undertaken by the customary homemaker, including such duties as preparation of meals, laundry, shopping, household chores.
- Transportation Services: Safe and reliable transportation to medical; dental; or mental health appointments, complex errands, financial institutions, stores, social events, seasonal festivities, or other excursions not listed.
- Companion Services: Emotional support, diversified conversation, social engagement, or educational and recreational companionship to reduce isolation and enhance well-being.
- Personal Care Services: Helping with daily activities such as getting in and out of the bathtub or shower, adjusting water temperatures, washing and drying portions of the body which are difficult for the client to reach, being available while the client is bathing, or washing & drying the client who is bed-bound. Personal care also includes helping the client put on and remove clothing, assisting with shaving, oral, hair, skin and nail care.
- Significant Change Monitoring: Documenting a sudden or major shift in the behavior or mood of a client that is inconsistent with the client’s diagnosis, or a deterioration in the client’s health status such as unplanned weight change, stroke, heart condition, enrollment in hospice, stage 2, 3 or 4 pressure sore, or recording vital signs.
- Overnight Monitoring: Providing supervision and support throughout the night for those clients who may be experiencing Sundowning. restlessness, or at risk of falls. We provide awake caregivers to alert of any sudden changes ensuring safety and a peace of mind for families.
- House Maintenance: Changing light bulbs, smoke detector or alarm batteries, hanging/removing pictures, potting plants, sweeping front/back porch or patio, watering plants, setting pest traps, bagging leaves, rearranging furniture (recliner, sofa, table), and more.
- Daily Supervision: Reminding clients to engage in activities of daily living, the self-administration of medications or observing, or providing verbal cuing to clients while they perform these activities.
- Light Housekeeping: Keeping the home tidy with basic cleaning of everyday tasks like vacuuming common areas, emptying trash, hoovering and mopping floors, sweeping stairs, cleaning bathrooms: toilet; bathtub/shower; sink; or mirrors, wiping down countertops, washing dishes, starting and emptying a dishwasher, changing linens & making bed, or doing laundry.
- Light Dusting: Maintaining a dust-free environment for a cleaner, healthier home dusting everyday use items like dresser tops, nightstands, or end tables.
- Simple Errands:Assisting with grocery shopping, checking the mail, picking up prescriptions, or other essential household items.
- Laundry Assistance: Helping with washing, drying, folding, and organizing clothing and linens.
- Meal Preparations: Preparing nutritious meals and snacks tailored to any dietary preference or need whether a regular; puree; or mechanical soft diet. We provide weekly or monthly menu planning to assist in maintaining nutrition and hydration, ensuring every meal is a comfortable and dignified experience.
- Grooming Assistance: Ensuring clients look and feel their best by providing assistance with oral care, shampooing & conditioning hair, shaving mustache or beards, nail filing & painting, toenail filing(non-diabetics only) and painting, foot baths & soaking, mini hand or foot massages, or skin care.
- Incontinence Care: Providing sensitive and discreet support with toileting and incontinence needs when using pullups or briefs, reminding the client about using the toilet, assisting to the bathroom, helping to undress to position on the commode, assisting with positioning on a bedpan, assisting with colostomy or catheter bags.
- Transferring/Repositioning: Providing verbal and physical cueing, physical assistance, or both while the client moves from one position to another using the following: a bed, chair, wheelchair, commode, bathtub, shower, or a standing position. Transfer/Repositioning can also include use of a mechanical lift device.
- Safety Precautions: Ensuring the home environment is safe, including fall prevention and other safety measures by suggesting the removal of tangible items.
- Deep Cleaning Options: Offering more thorough cleaning services to maintain a pristine and healthy living space, like vacuuming upholstery, cleaning baseboards, wiping window sills, cleaning blinds, cleaning window/door frames, cleaning inside oven or refrigerator, scale & grime removal, cleaning under furniture, behind appliances, cleaning ceiling fans or light fixtures, dusting lamp shades or individual decorative pieces, cleaning inside cabinets, under sinks, removing cobwebs, or cleaning a washing machine.
- Medication Assistance: Assistance with self-administration of medication or other tasks from a person unlicensed to practice nursing or medicine who is employed or under contract by Hand To Heart who will not be overseen by a licensed nurse but has received training with respect to assisting clients with the self-administration of medication or other tasks and is authorized to do so.
Medications include both legend and over-the-counter oral dosage forms, topical dosage forms, transdermal patches, and topical ophthalmic, otic, and nasal dosage forms including solutions, suspensions, sprays, and inhalers, assistance with a glucometer, assistance with anti- embolishomem stockings, assistance with applying and removing an oxygen cannula but not titrating the prescribed oxygen settings, assistance with a continuous positive airway pressure device but not with titrating the prescribed setting of the device. - Respite Care Services: Care Services provide temporary relief for primary caregivers offering trusted, compassionate care for your loved one, so you can take time to recharge with peace of mind.
- Organizing/Scheduling: We ensure clients have a structured, well-planned routine, helping them stay on top of daily tasks by assisting with appointment scheduling, recording appointments in a planner, reserving transportations, confirming appointment dates & tines with family and/or other professionals, canceling appointments, or bill paying.
- Feeding Assistance: Helping clients with or by cutting food, pouring beverages, or feeding clients who are unable to feed themselves by way of spoon/fork; straw; cup, or intake encouragement.
- Ambulation Assistance: Providing physical support and mobility to enable the client to move safely and confidently about within or outside of the client’s place of residence like holding the client’s hand, elbow, under the arm, or holding on to a support belt worn by the client to assist in providing stability or direction while the client ambulates.
- Alzheimer’s/Dementia Care: We provides specialized support for clients with Alzheimer’s disease and other related disorders. Focusing on activities specifically designed for people with cognitive impairment. We provide daily engagement and emotional well-being support to help each client maintain a quality of life with respect and dignity.
